The information is here to help you understand the type of service I provide and show you that I am committed to giving a first class service. Parent & Childminder Checklist

The following list contains some of the documents that may be available at the service. These should be shown to you as part of the initial interview, if not ask the childminder to see some of the items. This list is not an exhaustive; childminders will have varied and different documentation.

  1. Care Commission/OfSTED Registration Certificate
  2. Disclosure Scotland/CRB Check (for all adults within the household)
  3. Public Liability Insurance (normally from an association e.g. NCMA, SCMA, NICMA)
  4. Car Insurance Certificate showing Business Use
  5. Care Commission/OfSTED Inspection Report (unless newly registered)
  6. Standard Contract Documents
  7. Incident/Accident Records
  8. Medicine Records
  9. Attendance Register
  10. Permission Forms for:
